MCM Collaborative Psychotherapy LCSW PLLC is committed to safeguarding your privacy, especially with regard to any Protected Health Information (PHI) that you may reveal on our website. This page describes the details of how we use information collected on our website and how we’re ensuring the privacy of that information. When starting treatment with us, you will receive another document that reviews how we ensure the privacy of your clinical information, which meets established HIPAA standards.

Definitions

  10. Right for the help of supervisory authority – meaning that you have the right for the help of a supervisory authority and the right for other legal remedies such as claiming damages.

This information is what you have provided us via our contact form – this may include your e-mail address, name, phone number, and any other personal information that you entered into forms. This is mainly information that is necessary for connecting you with the services you are seeking.

We do not intend to collect or knowingly collect information from children. While we offer services for children, we do not target children themselves with our services.
